About The Position

UPMC Family Hospice is hiring a Home Care Aide for a casual position to cover Lycoming, Union, Northumberland, Snyder, Sullivan, Clinton, Columbia, parts of Centre, Montour counties. This role involves providing designated care under the direction of the primary service provider in the home to achieve identified goals, functioning as an extension of skilled nursing services and/or rehabilitation services in the home. The position requires 16 hours every pay period with rotating holidays and weekends, including a weekend rotation every 4th weekend.

Requirements

  • High School Diploma or GED
  • Minimum of 75 hours of Nurse Aide training
  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License
  • Act 33
  • Act 34
  • OAPSA

Nice To Haves

  • One (1) year experience
  • Certified Nurse Aide
  • If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.

Responsibilities

  • Maintain annual competency evaluation scores as determined by regulatory requirements.
  • Attend compliance training and adhere to the organization's standards of conduct, policies, and procedures.
  • Focus on customer service and continually strive to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Identify, develop, and participate in process improvement opportunities within the home health agency that will enhance the quality of services provided.
  • Perform duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  • Participate in all required agency meetings.
  • Communicate appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient’s condition to appropriate professional staff.
  • Maintain and complete documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
  • Provide safe and direct personal care to patients in the home setting under the direct supervision of an appropriate professional.
