About The Position

The Home Care Aide helps clients remain as independent as possible by aiding in the client’s home environment. This role involves supporting clients in their homes, facilitating communication, maintaining a safe environment, and preparing nutritious meals. The position also requires adherence to organizational policies, compliance training, and promoting exceptional customer service within the Salemtowne community.

Requirements

  • High school diploma or GED.
  • Active CPR certification.
  • Excellent interpersonal and interdisciplinary relationships.
  • High level of initiative and creative problem-solving ability.
  • Stellar customer service oriented.
  • Ability to model and ensure others are providing medical treatment in a manner that honors elders’ preferences and individuality.
  • Ability to be nimble to do any tasks needed, including those that may seem menial or unpleasant.
  • Proficient in recognizing early signs of changing conditions and uses all available and appropriate resources to resolve needs.
  • Practices and models person-centered approach.
  • Demonstrates tact and empathy.
  • Presents a professional appearance and demeanor.

Responsibilities

  • Records client information by making entries in the client care record (electronically); notifying agency nursing supervisor of changing or unusual conditions.
  • Helps client remain as independent as possible by aiding in their home environment.
  • Helps identify areas that can increase client independence in their home.
  • Helps client remain active and involved in the Salemtowne Community as client is able/or desires.
  • Facilitates communication between client and their families by use of technology.
  • Maintains a safe, secure, and healthy client environment by following nursing standards and procedures; changing linens, dusting, vacuuming; maintaining kitchen and bathroom facilities, handling personal laundry; and maintaining security.
  • Provides nutritious meals by following prescribed dietary requirements and nutrition standards, preparing, cooking, and serving meals and snacks.
  • Supports the organization's mission, vision, and values by becoming familiar and acting in accordance with company policies and procedures.
  • Promotes exceptional customer service by participation in Salemtowne Shines & fulfilling the commitments & values of the program.
  • Completes annual compliance and HIPAA training and exhibits behavior as set forth by the Code of Compliance in the performance of their duties.
  • Adheres to Salemtowne’s policies and procedures regarding the compliance program of the organization and is timely in reporting any possible concerns.
  • Follows all local, state, and federal regulations as they pertain to their position.
  • Adheres to the Salemtowne’s policies and procedures covering privacy and security in compliance with HIPAA regulations.
  • Performs other duties as assigned.
