About The Position

UPMC Home Health is hiring a Home Care Aide to join its team and support the Apollo area to the Fox Chapel area. This position is a Monday through Friday, 8:00A-4:30P role. Provide designated care under the direction of the primary service provider in the home to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.

Requirements

High School Diploma or GED
Minimum of 75 hours of Nurse Aide training
Automotive Insurance
Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
Driver's License
Act 33
Act 34
OAPSA

Nice To Haves

One (1) year experience
Certified Nurse Aide (CNA)

Responsibilities

Must maintain annual competency evaluation scores as determined by regulatory requirements
Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.
Proven ability to facilitate change.
Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
Participates in all required agency meetings.
Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
