About The Position

Bay Aging is seeking Home Care Aide-Personal Care Assistants (PCA) to deliver in-home care services to maintain the health and improve the quality of life of clients so they can live safely in the community. The role involves providing personal care, health and safety monitoring, and light housekeeping to clients. This position reports to the Registered Nurse (RN) Supervisor.

Requirements

  • Must be 18 years of age.
  • Must hold a Certificate of Completion course that meets the Department of Medical Assistance approved curriculum for a Personal Care Aide (PCA).
  • Must provide proof of current PPD vaccination or complete a Tuberculosis screening.
  • Working knowledge of HIPAA regulations.
  • Working knowledge of adult care practice and procedures.
  • Working knowledge of Bay Aging policies and procedures related to Home Care.
  • Knowledge of medical equipment usage and basic maintenance such as Hoyer lift, gait belt, wheelchairs, oxygen tanks, catheters, glucometer and any specialized equipment required by the care plan.
  • Ability to stay current in the knowledge of their profession by completing at least 12 hours of in-service annually.
  • Ability to speak, read and write English.
  • Ability to establish and maintain good relationships with clients and their families even in difficult circumstances.
  • Ability to communicate effectively with older adults suffering from dementia or Alzheimer’s disease.
  • Ability to communicate with clients’ families to ensure good continuity of care.
  • Ability to be considerate of care recipients and hold their personal information as confidential.
  • Ability to be observant of changes in client’s condition or environment.
  • Ability to stand, bend, stoop, pull, push wheelchairs and lift up to 25lbs unassisted.

Responsibilities

  • Follow the individual Care Plan developed by the RN Supervisor.
  • Assist with Activities of Daily Living (ADL) including bathing, dressing, grooming, toileting, transferring, eating, and ambulation.
  • Turn and change client position.
  • Provide assistance with Special maintenance.
  • Check vitals.
  • Supervise medication.
  • Assist with specialized care needs as assigned by the RN supervisor.
  • Provide supervisory care for clients who require constant monitoring.
  • Perform Independent Activities of Daily Living (IADL) including meal preparation.
  • Perform light housekeeping in the area of the home used by the client.
  • Perform laundry.
  • Accompany client to medical appointments, post-secondary school or in the community setting when assigned.
  • Provide assistance with errands outside the home.
  • Run errands within the local area for the client (e.g. grocery store, pharmacy, Post Office).
  • Monitor home environment for health and safety.
  • Document all services provided.
  • Complete Aide records each day.
  • Submit Aide records as directed.
  • Communicate all changes in client’s physical or mental condition to RN Supervisor including suspicions of exploitation, abuse or neglect.
  • Complete at least 12 hours of formal training annually.
  • Perform a limited number of routine tasks prescribed by clients’ care plan.
  • Work independently throughout the day and prioritize work each day.
  • Report to supervisor any change in the client’s environment, physical or mental condition.
