About The Position

Deliver in-home care services to Bay Aging clients so they can live safely in the community. Provide personal care, health and safety monitoring and light housekeeping. Introduce and coach new Personal Care Aides in the field. Perform administrative work in the office to complete a full-time schedule every week. Report to the Registered Nurse (RN) Supervisor.

Requirements

Must be 18 years of age
An NA must hold a certificate of completion for a state-approved education program that includes both instruction on the principles of nursing and supervised clinical work curriculum or maintains a licensure as a CNA issued by the Virginia Board of Nursing.
Must provide proof of current PPD vaccination or complete a Tuberculosis screening.

Nice To Haves

Working knowledge of HIPAA regulations.
Working knowledge of Adult care practice and procedures.
Working knowledge of Bay Aging policies and procedures related to Home Care.
Knowledge of medical equipment on usage and basic maintenance such as Hoyer lift, gait belt, wheelchairs, oxygen tanks, catheters, glucometers and any specialized equipment required by the care plan.
Ability to stand, bend, stoop, pull, push wheelchairs and lift up to 25lbs unassisted.

Responsibilities

Assist with Activities of Daily Living (ADL) - Bathing, dressing and grooming, Toileting, Transferring, Assist with eating, Ambulation, Turn and change position
Provide assistance with Special maintenance - Check vitals, Supervise medication, Assist with specialized care need as assigned by the RN supervisor, Provide supervisory care for clients who require constant monitoring.
Perform Independent Activities of Daily Living (IADL) - Meal preparation, Light housekeeping in the area of the home used by the client, Laundry, Accompany client to medical appointments, post-secondary school or in the community setting when assigned.
Provide assistance with errands outside the home. Run errands within the local area for the client (e.g. grocery store, pharmacy, Post Office)
Monitor home environment for health and safety.
Document all services provided. Complete Aide records each day. Submit Aide records as directed.
Communicate all changes in client’s physical or mental condition to RN Supervisor including suspicions of exploitation, abuse or neglect.
Complete at least 12 hours of formal training annually.
Provide on-the job training and coaching to new PCAs.
Work under the direction of the RN Supervisor or Scheduling Coordinator, to perform office administrative duties including but not limited to filing charts and making copies.
