About The Position

Bay Aging is seeking part-time Home Care Aide Nursing Assistants for Colonial Beach. This role involves delivering in-home care services to Bay Aging clients to enable them to live safely in the community. Key responsibilities include providing personal care, health and safety monitoring, and light housekeeping. The position also requires introducing and coaching new Personal Care Aides in the field, and performing administrative work in the office to complete a full-time schedule every week. The Home Care Aide Nursing Assistant reports directly to the Registered Nurse (RN) Supervisor.

Requirements

  • Must be 18 years of age.
  • Hold a Certificate of Completion for a Personal Care Aide (PCA) course that meets the Department of Medical Assistance approved curriculum.
  • Must provide proof of current PPD vaccination or complete a Tuberculosis screening.
  • Become certified in CPR.
  • Working knowledge of HIPAA regulations.
  • Working knowledge of Adult care practice and procedures.
  • Working knowledge of Bay Aging policies and procedures related to Home Care.
  • Knowledge of medical equipment usage and basic maintenance such as Hoyer lift, gait belt, wheelchairs, oxygen tanks, catheters, glucometer and any specialized equipment required by the care plan.
  • Stay current in the knowledge of their profession by completing at least 12 hours of in-service annually.
  • Ability to speak, read and write English.
  • Ability to establish and maintain good relationships with clients and their families even in difficult circumstances.
  • Ability to communicate effectively with older adults suffering from dementia or Alzheimer’s disease.
  • Ability to communicate with clients’ families to ensure good continuity of care.
  • Ability to be considerate of care recipients and hold their personal information as confidential.
  • Ability to be observant of changes in client’s condition or environment.
  • Ability to stand, bend, stoop, pull, push wheelchairs and lift up to 25lbs unassisted.

Responsibilities

  • Follow the individual Care Plan developed by the RN Supervisor.
  • Assist with Activities of Daily Living (ADL) including bathing, dressing and grooming, toileting, transferring, assisting with eating, ambulation, and turning and changing position.
  • Provide assistance with Special maintenance including checking vitals, supervising medication, assisting with specialized care needs as assigned by the RN supervisor, and providing supervisory care for clients who require constant monitoring.
  • Perform Independent Activities of Daily Living (IADL) including meal preparation, light housekeeping in the area of the home used by the client, and laundry.
  • Accompany client to medical appointments, post-secondary school or in the community setting when assigned.
  • Provide assistance with errands outside the home, running errands within the local area for the client (e.g. grocery store, pharmacy, Post Office).
  • Monitor home environment for health and safety.
  • Document all services provided and complete Aide records each day.
  • Submit Aide records as directed.
  • Communicate all changes in client’s physical or mental condition to RN Supervisor including suspicions of exploitation, abuse or neglect.
  • Complete at least 12 hours of formal training annually.
  • Provide on-the job training and coaching to new PCAs.
  • Perform office administrative duties including but not limited to filing charts and making copies, under the direction of the RN Supervisor or Scheduling Coordinator.
  • Perform a limited number of routine tasks prescribed by clients’ care plan.
  • Work independently throughout the day.
  • Report to supervisor any change in the client’s environment, physical or mental condition.
