About The Position

Established in 1980, the Greater Lawrence Family Health Center (GLFHC) is a multi-site, mission-driven, non-profit organization employing over 700 staff whose primary focus is providing the highest quality patient care to residents throughout the Merrimack Valley. Nationally recognized as a leader in community medicine — family practice, pediatrics, internal medicine, and geriatrics — GLFHC operates multiple clinical sites throughout the service area and serves as the sponsoring organization for the Lawrence Family Medicine Residency Program. Position Summary: The Health Information Management Scan Clerk provides support for providers and clinical staff by scanning outside reports and internal handwritten reports into the electronic medical record (EMR) within 48 hours of receipt. This role is responsible for accurately indexing reports, assigning appropriate document types, verifying patient information, and ensuring records are complete, legible, and routed appropriately. Although each Health Information Management Clerk may be assigned to a specific function, all Clerks are expected to understand and support the overall functions of the HIM Department.

Requirements

  • Computer experience required
  • Strong communication, telephone, and customer service skills
  • Excellent attention to detail and documentation accuracy
  • Ability to work in a fast-paced or stressful environment while maintaining composure
  • Ability to function with minimal supervision
  • Ability to apply independent judgment to issues with potential impact on the clinic
  • Understanding of medical terminology required

Responsibilities

  • Provide customer service support to patients and staff requesting health information
  • Scan and index outside reports and internal handwritten reports into the EMR
  • Verify patient identifying information, including name, date of birth, and medical record number
  • Ensure all pages belong to the correct patient, report, and are in proper order
  • Confirm forms are completed and signed by the appropriate clinician or individual before scanning/indexing
  • Review scanned documents for legibility before final saving in the EMR
  • Assign appropriate document types to all scanned reports
  • Route documents to the appropriate clinician’s electronic desktop for signature, as required
  • Complete open referral orders in the EMR for consult and radiology reports indexed
  • Prepare and maintain scanning/indexing logs and other tracking tools as required
  • Answer phones and voicemails in a timely manner
  • Cover clinical site HIM offices as needed for staff absences
  • Maintain strict confidentiality of patient medical information
  • Maintain knowledge of release of information rules and regulations
  • Participate in quality improvement activities related to individual, team, and organizational performance
  • Travel between sites as needed
  • Perform additional duties as assigned
