About The Position

We are seeking a highly organized, detail-oriented, and dependable HIM / Medical Records Custodian to join our busy 10-physician care center. This position is responsible for maintaining the accuracy, integrity, confidentiality, and accessibility of patient health records while supporting physicians and staff with timely medical-record services. The ideal candidate understands the importance of accurate health information, has strong knowledge of HIPAA and medical-record requirements, and can effectively manage competing priorities in a fast-paced medical practice. This role ensures patient records are complete, accurate, properly filed, and readily accessible to authorized staff. It involves processing medical-record requests from various entities, monitoring incoming health information, maintaining strict confidentiality, and communicating professionally with patients, providers, and other authorized requestors. Other HIM and medical-record duties as assigned.

Requirements

High School diploma/GED or equivalent
Comply with company policies, procedures, practices and business ethics guidelines.
Complete job required training.
Comply with all applicable laws and regulations, (e.g. HIPAA, Stark, OSHA, employment laws, etc.)
Demonstrate prompt and reliable attendance
Work in the surgery center during business hours
Travel independently throughout the surgery center (which may include movement from floor to floor); frequent bending, lifting, standing, stooping or sitting for long periods of time may be required
Work at an efficient and productive pace, handle interruptions appropriately and meet deadlines
Communicate and interact in a respectful and professional manner
Prioritize workload while being flexible to meet the expectations of the daily operations
Apply principles of logical thinking to define problems, establish facts, and draw valid conclusions
Understand and execute a variety of instructions
Effectively operate equipment and communicate on and operate the phone system
Work independently with minimal supervision

Nice To Haves

Medical terminology knowledge preferred
Two years of experience in related field preferred
Travel to other work locations, if required

Responsibilities

Maintaining the accuracy, integrity, confidentiality, and accessibility of patient health records.
Supporting physicians and staff with timely medical-record services.
Ensuring patient records are complete, accurate, properly filed, and readily accessible to authorized staff.
Processing medical-record requests from patients, attorneys, insurance companies, other healthcare providers, and authorized third parties.
Responding to record requests within required timeframes while following appropriate authorization and release procedures.
Monitoring and processing incoming medical records, referrals, reports, correspondence, and other health information.
Maintaining strict confidentiality of protected health information and following HIPAA and other applicable privacy requirements.
Communicating professionally with patients, providers, staff, outside medical facilities, and other authorized requestors.
Maintaining accurate tracking file for receipt of dictation and signatures.
Communicating with physicians and their care teams to ensure timely receipt of dictation and signatures.
Meeting active annual goals for the medical records department with chart audits and compliance.
Verifying and operating within state mandated requirements for medical records requests.
Issuing invoices and tracking payments from entities requesting medical records.
Retrieving appropriate chart documentation as indicated by the facility policy for returning patients.
Maintaining confidentiality and compliance measures for medical records maintenance at the center.
Tracking completion of pathology reports.
Assisting with the registration of surgery patients including obtaining all appropriate insurance information and signatures from patients.
Assisting in retrieval hospital/ER records for all transfer patients and/or cases.
Reporting occurrences through the established reporting process related to employee and/or patient/visitor events.
Fostering the developmental and educational process of self, colleagues, trainees and students (includes functioning as a preceptor as needed).
Participating in acceptable continuing education, fostering career and skill development including meeting departmental continuing education requirements.
Assisting in managing controllable costs by the proper, non-wasteful use of Surgery Center resources.
Performing any and all other duties as assigned including those assigned to Patient Services Representative.
