About The Position

Coordinates health records processing functions; Qualitative and quantitative analysis of record content and indexing images to the EMR. Manages Follow My Health portal. Responsible for collecting, maintaining and reporting various performance improvement indicators for the HIM Department and clinical areas. Validates charge data entry by clinicians. Serves as a liaison to the Unit secretaries for HIM content and processes. Ensures confidentiality of patient information.

Requirements

  • Minimum Associate Degree or will consider comparable work experience.
  • Minimum two-three years HIM experience preferred; will consider past experience and skill set.
  • Medical Terminology, Transcription & Release of Information, HIPAA release guidelines and general HIPAA privacy requirements.
  • Knowledge of data analysis techniques, data aggregation and data display.
  • Knowledge of and/or ability to learn legal aspects of medical records.
  • Good organizational skills, with orientation to detail and accuracy; excellent oral and written communications skills; ability to interact positively with physicians, administration, hospital staff and the public in a professional manner.
  • Ability to work independently, with little supervision.
  • Excellent communication skills, Microsoft office, excel, PowerPoint and telephone skills required.
  • EMR experience required.

Responsibilities

  • Retrieves discharged records, assembles and scans in any documents not previously scanned to the EMR, performs qualitative and quantitative review to assure all required documents are present and completed.
  • Performs qualitative analysis on all scanned documents from all areas that scan patient information, reviewing quality, and releasing to index to the EMR throughout the day, assuring timely availability of documents within 4 hours of scanning. Maintains quality report for Corporate Director review monthly. Assists with training all areas responsible for scanning as needed.
  • Communicates with physicians and/or their office staff regarding incomplete records for content, history and physicals, discharge summaries and Signatures. Communicates deficiency within the 3 days after patient discharge. Ensures that transcribed documents are uploaded to the EMR timely.
  • Monitors clinical content using defined quality metric spreadsheets to capture cumulative data, prepares reports monthly for Corporate HIM Director. Monitors Lab and Radiology ordering samples to assure orders are complete, documentation present in the EMR. Submits monthly report to Corporate HIM Director.
  • Provides customer support for Follow my Health patient portal, reviewing logs and resending invitations as needed. Answers questions from patients and provides assistance to the patient to engage with their portal. Uses portable scanning/PC station to assist with patient signup in ambulatory and inpatient units.
  • Reviews duplicate charge list daily, confirming documentation supports the charge entered by the provider, record and delete any charges not supported. Review findings with Lead weekly.
  • Ensures that confidential patient information is released only to authorized individuals and requesters. May assist with the processing of requests daily or for coverage to ensure that timeliness meets established standards. Follows all hospital and HIPAA Privacy guidelines.
  • Maintains off site storage process recalling records as needed and returning to storage promptly. Continues to send remaining paper records for files not scanned to EMR prior to 2015 which may be part of overflow. Assists others within the organization that utilizes offsite storage.
  • Provides educational support to patient care areas regarding the proper maintenance of medical records, record format for record pick-up, dictation instructions, and/or instructions on how to prepare records for record storage vendor.
  • Maintains knowledge of Federal, State, Regulatory and Accreditation standards of documentation regulations and guidelines to ensure understanding and quality of documentation requirements.
