Requirements

High School\GED degree required (Associate\Bachelor preferred)
3-5 years of experience in health care settings, ideally in the areas of direct patient care, case management, care coordination, patient outreach, front desk\clinic reception, and\or quality improvement
Ability to read, write and speak English.
Able to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Knowledge of electronic medical records (experience in Epic preferred) , and Microsoft Office Suite including Excel, MS Word, PowerPoint, Outlook etc. Proficient in data analysis and manipulation (Excel summary tables, charts, pivot tables)

Responsibilities

The High Value Care Coordinator monitors value/quality based reimbursement programs offered by medical group commercial payers and CMS and works in collaboration with the Practice Administrator and/or Practice Manager at each location to close gaps and increase value based reimbursements.
