About The Position

EMPACT- Suicide Prevention Center, part of the La Frontera family, is seeking High Needs Case Managers to work with children and families requiring a high level of service provision, contact, and coordination of treatment. This position offers direct service crisis intervention, support, transportation, and education to families and children, independently or in conjunction with other in-home client services. The role involves managing a caseload of 15 families to ensure intensive service delivery.

Requirements

  • Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
  • Behavioral health experience is required.
  • Knowledge of the CFT process and Arizona 12 Principles.
  • Ability to be flexible and problem-solve.
  • Ability to use creativity in service delivery approach.
  • Must have a Fingerprint clearance card or be eligible to obtain one.
  • Must have a valid AZ driver's license with no major infractions in the past three years.
  • Must have regular access to reliable transportation.
  • Must be at least 21 years of age at the time of employment.
  • Demonstrated ability to remain calm in crisis situations.
  • Ability to work independently and cooperatively.
  • Ability to recognize personal limitations.
  • Ability to relate positively and with empathy to all persons regardless of age, race, creed, gender, or sexual orientation.
  • Maintain professional conduct consistent with ethical professional practice and agency policies and procedures.
  • Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners.
  • If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required.
  • At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.
  • Staff members who provide direct client services must be at least 21 years old.
  • All employees will have a minimum of a high school diploma or equivalent.

Nice To Haves

  • Previous experience working with children is preferred.
  • Bi-lingual (Spanish) is a plus.

Responsibilities

  • Provide direct in-home, in-office, in-school, and community services, including crisis intervention, transportation, maintaining client stabilization, additional client supervision, parent training, and support to maintain a child in the home.
  • Facilitate, schedule, and provide follow-up on family, youth, and adult team meetings (ACT or CFT), completing written documentation and distributing it to all team members.
  • Engage and invite natural supports as identified by family, youth, and adults.
  • Make referrals to collaborating partners and other community services based on client needs.
  • Deliver at least 40 hours of work per week, with a goal of 30 hours of direct client contact, including case management.
  • Ensure clinical documentation meets established monthly/annually standards.
  • Complete all tasks within agreed-upon timelines.
  • Work a flexible schedule based on the needs of children and families, which may include evening or weekend hours.
  • Attend meetings and training as requested.
  • Support services to families, youth, and adults in collaboration with each child and family, adhering to the 12 Arizona Principles.
  • Ensure continuity of care in comprehensive services and assist additional team members in coordinating communication with other agency departments or external stakeholders.
  • Develop and maintain effective working relationships with related state, community mental health, and contracting organizations.
