About The Position

The ATLAS Project, an initiative of the Mayor’s Office of Neighborhood Safety in partnership with United Way of NYC, aims to reduce community and interpersonal violence by engaging people in a change process that nurtures hope and strengthens relationships. Atlas meets people at times of crisis, such as an interpersonal or family conflict, a recent arrest, or a recent court case and connects them to free community-based therapeutic programming. The therapeutic approaches offered through Atlas are evidence-informed, meaning they have strong evidence of effectiveness in promoting personal change and preventing future involvement in violence or incarceration. The approaches are culturally competent, trauma-informed, and deeply respectful of the strength, wisdom, and personal agency that resides in people. ATLAS seeks to enhance peace and prevent violence in New York City through transformative investments in families, community-based organizations, and in the systems meant to maintain accountability and public safety. We recognize the impact “systems” have on the lives of marginalized persons; keeping them in a cycle of involvement that impacts their freedom, self-efficacy, and dignity. Atlas is an investment in the agency and self-reliance of people affected by system involvement and in the community partners that serve them. Rising Ground is seeking a resourceful, community-minded Case Manager to join a specialized, high-intensity team serving youth and families with complex needs. The Case Manager will work closely with FFT Therapists, Credible Messengers, and the Lead Therapist to make sure youth and families have the practical support and resources they need to engage in services and make progress. The Case Manager will take an active, hands-on approach to supporting families. This includes identifying resources, connecting families to services, accompanying them to appointments, attending school meetings, supporting discharge planning, and helping address needs within the family—including the needs of other children in the home. This role also requires strong community engagement. The Case Manager will build relationships with schools, community organizations, service providers, and other local partners to identify resources and create opportunities that can benefit youth and families. They will also respond to emerging community needs and develop creative, practical solutions that help youth stay connected to school, family, community, and positive supports. The Case Manager will help assess family needs, support engagement, remove barriers to participation, and provide the therapist with the administrative and case management support needed to focus on clinical work. This is a highly mobile, relationship-driven role that requires flexibility, creativity, strong follow-through, and a willingness to go beyond traditional case management to meet families where they are.

Requirements

Bachelor’s degree in social work, human services, psychology, sociology, education, public health, or a related field preferred; relevant community-based experience will be strongly considered.
At least two years of experience working with youth and families, particularly those impacted by violence, juvenile justice, criminal legal system involvement, or other complex systems.
Strong knowledge of NYC schools and the Department of Education, juvenile/criminal legal systems, and community-based resources available to youth and families.
Demonstrated ability to build authentic relationships with youth and families who may be difficult to engage, resistant to services, or distrustful of systems.
Strong resource navigation, advocacy, and problem-solving skills, including the ability to identify creative and nontraditional supports that address the needs of youth and families.
Strengths-based, family-centered approach and belief in the capacity of youth and families to grow, change, and develop their own solutions.
Ability to work effectively in homes, schools, community settings, and other environments; engage multiple systems and community partners; and collaborate as part of a multidisciplinary team.
Strong self-awareness, cultural humility, professional boundaries, and ability to remain calm, flexible, and effective when managing stress, conflict, and changing circumstances.
Ability to manage multiple cases and responsibilities while maintaining timely documentation and adherence to FFCM/FFT expectations.
Willingness to participate in FFCM/FFT training, supervision, consultation, and ongoing professional development.
Willingness to work flexible hours, including evenings and other nontraditional hours, based on family needs and availability, and travel throughout NYC.
Work-related travel expenses will be reimbursed by the agency.
Demonstrated cultural humility and ability to work respectfully and effectively with people from diverse backgrounds and lived experiences, including LGBTQIA+ individuals and people and communities who have been historically marginalized, oppressed, or disproportionately impacted by social, educational, and legal systems.
Authorized to work in the U.S.
Ability to work in-person in New York City (NYC)
Ability to travel to other Rising Ground Sites (if required)
Ability to travel within the community and do visits

Responsibilities

Maintain a caseload of 10–15 youth and families and provide Functional Family Case Management (FFCM) support.
Participate in required FFCM/FFT training, supervision, consultation, and team meetings and follow the model as trained.
Complete assessments to understand the needs, strengths, goals, and challenges of youth and families.
Provide hands-on support in homes, schools, the community, and other settings based on family needs.
Identify and connect youth and families to services, resources, opportunities, and other supports.
Accompany youth and families to appointments, school meetings, and other important activities when needed.
Assess the needs of other children and family members in the home and connect them to appropriate supports.
Support engagement in FFT by helping families address practical needs and barriers to participating in services.
Work closely with FFT Therapists and Credible Messengers to coordinate support, share information, and help families make progress.
Support discharge planning and help families build connections to ongoing services and community supports.
Build relationships with schools, community organizations, service providers, and other partners to expand resources and opportunities for families.
Find creative and nontraditional solutions to meet the needs of youth and families and respond to emerging community needs.
Participate in community outreach, community responses, and other activities that strengthen relationships with youth, families, and the community.
Maintain timely and accurate documentation, including required FFCM/FFT case notes within 48 hours.
Maintain a flexible schedule and travel throughout NYC based on the needs of youth and families.
Other related duties as assigned by supervisor.
