About The Position

This role provides patient-centered leadership to clinical professional and paraprofessional staff, sharing direct supervision of RNs, LPNs, and Home Health Aides. Key responsibilities include making patient and personnel assignments, coordinating patient care, and ensuring continuous assessment of patient needs. The position assures the development, implementation, and updates to individualized plans of care, utilizing quality improvement concepts and evidence-based models of practice. Under the direction of the Clinical Operations Director, this role shares responsibility for the oversight of the weekend/overnight/holiday clinical team. It involves educating and mentoring staff, interfacing with outside agencies and families, and conducting interdisciplinary team meetings. Quality Improvement activities are conducted through case conferences, clinical record review, billing review, on-site home visits, staff supervisory visits, and team meetings. OASIS review and Care plan (485) review and completion are conducted in collaboration with the Coding and Compliance Director and the Director of Education. In conjunction with the Director of Education, the role ensures successful orientation and preparedness of new hires, including timely skills validation. Periodic review of care outcomes, client goals, and care plan accuracy is performed. The role supervises and evaluates assigned staff performance to meet regulatory and accrediting requirements, assists staff with goal setting and relationships, and reviews staffing levels with recommendations for changes under the guidance of the Clinical Operations Director. Additional responsibilities may be assigned.

Requirements

  • Current license to practice as a professional registered nurse in the State of Connecticut.
  • Three to five years of registered nursing experience with a minimum of 2 years in Community Health/Home Care Nursing.
  • Competent knowledge of Medicare Conditions of Participation and Department of Public Health regulations for home care.

Nice To Haves

  • Two years of experience in supervisory or other administrative nursing positions preferred.
  • Master’s Degree in Community Health Nursing or related field preferred.

Responsibilities

  • Under the direction of the Clinical Operations Director, share responsibility for the oversight of the weekend/overnight/holiday clinical team, including LPNs.
  • Educate and mentor staff in the management of complex/difficult cases.
  • Interface with outside agencies, patient families, referral sources, and liaison staff as needed.
  • Conduct and lead interdisciplinary team meetings on a regular basis.
  • Conduct Quality Improvement activities for professional home care services through case conferences, clinical record review, billing review, on-site home visits (patient assessments), on-site staff supervisory visits, and team meetings.
  • Ensure the successful orientation and preparedness of all new hires under organizational supervision, including timely skills validation.
  • Review periodically with assigned clinicians the outcomes of care, appropriateness of client goals, and achievement of client goals.
  • Supervise and evaluate the performance of all assigned staff to fulfill regulatory and accrediting requirements.
  • Assist assigned staff in setting individual goals and managing intra- and interagency relationships.
  • Review staffing levels, make recommendations, and implement changes under the guidance of the Clinical Operations Director.
  • Identify problems affecting patient care and clinical management, and develop solutions mutually with staff and the Clinical Operations Director.
  • Ensure professional development of staff and completion of necessary continuing education requirements through in-service education and the provision of a climate conducive to the sharing of knowledge and expertise between peers.
  • Communicate ideas and concerns of management to and from staff and elicit discussion.
  • Assist with organizational changes by incorporating procedures into daily action.
  • Participate in recruiting, hiring, and performance improvement plans for employees.
