About The Position
All About You Collaborative Health Care Services LLC, is a Nurse owned and operated, transitional home care agency seeking exceptional people to join their team to help transition patients from hospital/facility to home. They are a preferred provider for several local hospitals/facilities and Assisted Living Facilities. The company is experiencing exciting growth and is looking for candidates who can work in Woodbury, Oxford and surrounding towns part time or full time and are available on weekends and evenings. Nice To Haves
- Certificate from a state sponsored homemaker/home health aide training and competence evaluation program.
- Certificate from nurse’s aide training and competency evaluation program as defined in Section 19-13-D8T (A) of the Connecticut Public Health Department of Regulations.
Responsibilities
- Assist the patient with personal care activities including bathing, oral hygiene, feeding and dressing.
- Assisting the patient with exercises, ambulation, transfer activities and medications that are ordinarily self-administered.
- Take and record temperature.
- Observes and reports patient status.
- Records care provided.
- Performs activities under special circumstances after training and observation by a Registered Nurse.
