• MUST HAVE HHA CERTIFICATE (OR PASS HHA TEST)
• MUST PROVIDE A COPY OF A VALID CPR CARD.
• MUST PROVIDE A COPY OF A NEG TB SKIN TEST RESULTS or CURRENT CHEST XRAY.
• MUST HAVE LEVEL 2 BACKGROUND CLEARANCE.
• MUST HAVE RELIABLE TRANSPORTATION.
• MUST BE ABLE TO PROVIDE PROOF OF CAR INSURANCE.
