About The Position
This is not a typical social work or nursing role. Hera is dedicated care management in the community for older adults, covered by Medicare. Every family gets matched with a dedicated Hero, who has the tools, time, and autonomy to help. It’s not a 15 min monthly check-in call. It’s real, ongoing support where you see things through and families call you their Hero. Heroes have autonomy to manage their own client load, work remotely from home with a flexible schedule, and use delightful technology built for them. They are never alone on a case with supportive training and community of other Heroes. As an early-stage, fast-growing company, Heroes joining now are shaping the company.
Requirements
- Must be located in the United States
- Tech-savvy and excited by quickly evolving environments and leveraging AI to supercharge your work
- Creative problem solver who wants a job that looks different every day
Nice To Haves
- Hold an LMSW, LCSW, or RN
- Experience in geriatric care management, hospice, palliative care, discharge planning, rehab, or hospital social work
Responsibilities
