About The Position

The Hepatology APP will see patients in an office clinic setting, documenting and creating orders in an electronic medical records system. This role involves rotating between two sites of care: Riverside Liver Clinic and OGG. The APP will coordinate care plans with primary care physicians and specialists, assess patients' medical, physical, and psychosocial status, and obtain relevant health and medical histories. Responsibilities include identifying medical and health risks, updating health status, developing patient education plans, coordinating discharge plans, and prescribing as needed in accordance with state licensure and physician agreements. The APP will also review and interpret fibroscan results, draft patient communications, assist physicians, and handle patient and physician calls. Secondary functions include maintaining patient privacy and confidentiality, and performing other tasks as requested by management.

Requirements

  • Must have current Nurse Practitioner License from the Ohio Board of Nursing and Certificate to Prescribe
  • Must have current BLS certification
  • Must have prior liver experience or 3 years of APP experience
  • Maintain annual PPD skin test/TB Screening Form

Responsibilities

  • Coordinate discharge plan and reassess the plan as necessary
  • Prescribe as needed in accordance with the current State Licensure and physician’s collaborative agreement
  • Review and interpret fibroscan results and draft patient communications
  • Assist physicians as requested/triage
  • Receive and return patient & physician calls as needed
  • Perform any follow up
  • Maintain patients’ rights of privacy and confidentiality
  • Perform any other tasks as requested by the physician, executive director, human resource manager, and/or the office supervisor
