The Team Coordinator assists with providing coordination of the intake and scheduling process for the Help at Home program to ensure quality of care delivery in a fiscally responsible manner. This role works under the direct supervision of the Director of Hospice and confers with Hospice and Home Health Team Managers regarding episodic management, centralized scheduling, quality improvement, patient satisfaction, and readmission initiatives.
