About The Position

The HEDIS Coordinator will manage patient panel oversight, track key empanelment metrics, and conduct staff training. This role supports scheduling, communication, and care coordination. It requires a detail-oriented professional with strong interpersonal skills and a commitment to delivering patient-centered care.

Requirements

  • Graduate of an accredited medical assistant program.
  • Certified Medical Assistant (CMA) Certification.
  • CPR and first aid certification
  • Ability to utilize computers, computer software/applications, and Microsoft Office applications (Excel, Outlook, PowerPoint, Word, etc.) with basic – intermediate proficiency.
  • Ability to communicate effectively with individuals within all levels of the organization, providers, patients, patient family members, and others.

Responsibilities

  • Manage and coordinate patient panels, including scheduling appointments, follow-ups, and tracking patient engagement.
  • Assist in onboarding new patients, ensuring accurate collection and maintenance of medical records.
  • Communicate with patients about their health needs, upcoming appointments, and personalized care plans.
  • Monitor patient data and clinical outcomes to identify areas for improvement in care delivery.
  • Handle administrative duties such as insurance verification, eligibility and patient registration.
  • Collaborate with clinical staff to gather data and verify the accuracy of patient records and documentation.
  • Stay informed about changes in HEDIS requirements and healthcare regulations to ensure ongoing compliance.
  • Facilitate training and education for staff on HEDIS measures and quality improvement initiatives.
  • Assisting members in scheduling appointments and completing preventive health services in accordance with HEDIS/STARS measures.
  • Stay up to date on healthcare quality regulations and standards to ensure compliance and adherence to best practices.
  • Conduct outreach for patients with chronic conditions to ensure they receive necessary services.
  • Contribute to a positive patient experience by providing courteous telephone interactions and appropriately referring cases to clinical staff when necessary.
  • Review patient medical charts to verify the provision of necessary services and identify areas for quality improvement.
  • Document and notify relevant personnel of any data inconsistencies discovered during patient record reviews.
  • Perform all job functions in accordance with federal, state, and local laws, as well as company policies and procedures.
