In conjunction with the Heart Failure team, this role assumes responsibility for the assessment, coordination, and education of Heart Failure patients in both inpatient and outpatient settings. The position assists in delivering high-quality, efficient medical care to patients within the Heart Failure Clinical Excellence Program at the Temple Heart and Vascular Institute. Responsibilities include managing complex heart failure patients, supporting physicians and advanced practitioners with patient care management, conducting assessments, interventions, referrals, documentation, telephone triage, and pre-certification registration processes. The role also involves providing ongoing health education and counseling, promptly reporting and following up on abnormal test results, and maintaining follow-up logs for record-keeping. Travel to other TUH Campuses may be required. Successful completion of the CHFN or CHFN-K - Heart Failure Nursing Certification is required within two years of the hire date.
