Healthcare Navigators connect Veterans with VA health care benefits and community health care services. They provide case management, care coordination, health education, interdisciplinary collaboration, consultation, and administrative support. Healthcare Navigators work closely with each Veteran’s primary care provider and assigned interdisciplinary treatment team. This position serves as the primary liaison between HAV (Homes for All Veterans) and VA or community health care systems. Healthcare Navigators support Veterans with complex needs who require assistance accessing health care services or following health care plans. They collaborate with assigned multidisciplinary teams, including medical, nursing, administrative, and case management personnel, to address these needs and provide timely, appropriate, equitable, and Veteran-centered care. The Healthcare Navigator serves Veterans across the Denver Metro area and Northern Colorado, including Boulder, Longmont, and Greeley, through a combination of home-based, community-based, and office-based services. This highly collaborative role requires regular travel to meet clients where they are, fostering meaningful connections and supporting access to healthcare and related resources. This position generally operates during standard business hours, Monday through Friday, with the flexibility to work occasional evenings and weekends to effectively meet client and program needs.
