The Healthcare Manager is responsible for supporting patients with high-risk health conditions as they navigate the healthcare system. The role promotes patient empowerment by serving as an educator, resource, and advocate for patients and their families to help maximize quality of life. The Healthcare Manager collaborates with multidisciplinary care teams, including physicians, nurses, pharmacists, laboratory technologists, social workers, and educators, and serves as a resource for clinic staff. Working in a less structured, self-directed environment, the Healthcare Manager performs nursing duties within the scope of an RN license in the applicable state. The position supports longitudinal care for patients with chronic conditions and transitions from inpatient to outpatient care. It involves coordinating care plans, documenting in the electronic medical record, participating in team meetings, preparing reports, and maintaining nursing competence and knowledge of current evidence-based practices. The role may also include clinical tasks within the nurse’s scope of practice.
