About The Position

This is a ground-floor opportunity to join a growing care navigation program at a moment when your contribution will directly shape how it scales. As a Healthcare Intake Coordinator, you are the first human voice a referred or prospective patient hears. Your job is to conduct proactive telephonic outreach, introduce patients to a program designed to support their health, accurately complete clinical intake questionnaires, and ensure they seamlessly transition into active care. This is an elevated, metrics-driven patient outreach role. The right person brings warmth and persistent follow-through in equal measure, is energized by meaningful conversations with older adults, and understands that a successful intake isn't just a verbal agreement—it’s an onboarded patient who shows up to their initial appointment. This program is early-stage and being built in real time. Processes will evolve and priorities will shift. Requirements

  • Must reside in Alberta, Canada and be legally authorized to work without sponsorship.
  • Holds a Medical Office Assistant (MOA) Certificate/Diploma from an accredited post-secondary institution, or equivalent verified Canadian healthcare administration experience.
  • 1+ years of patient-facing healthcare experience (e.g., medical clinic front office, patient registration, care coordination, enrollment or telehealth intake) where medical terminology and clinical data collection were daily requirements.
  • Proven experience in high-volume outbound phone environments (such as clinical outreach, patient scheduling lines, or health services engagement) with a track record of driving patient follow-through.
  • Highly comfortable navigating multiple software systems and electronic health records (EHRs) simultaneously while managing a live, empathetic conversation.
  • You possess a "builder" mentality—you are comfortable with ambiguity, love creating order out of chaos, and easily adapt to shifting processes.

  • Previous remote experience is strongly preferred.

Responsibilities

  • Manage a structured outbound referral queue, contacting prospective program participants with consistency, warmth, and professionalism.
  • Guide patients through plain-language explanations of program benefits. Complete and document standardized clinical intake questionnaires in real time.
  • Build rapid rapport with older adults who may be skeptical, confused, or reluctant, using empathy and clear communication to overcome barriers to care.
  • Strictly follow compliance protocols to obtain required verbal consents individually and sequentially before proceeding to scheduling.
  • Coordinate and book the patient's initiating clinical appointment immediately upon successful intake.
  • Log all outcomes, clinical metrics, and patient details across multiple digital health platforms simultaneously during the call.
