We are looking for a detail-oriented Healthcare Coordinator to join our call center, "The Pit," as the primary point of contact for residents, families, and healthcare providers. In this role, you will act as a vital liaison, ensuring onsite physician visits and ancillary services are executed with precision while minimizing disruptions to facility operations. We are looking for a proactive problem-solver who can navigate complex care needs with empathy and high-level organizational skills.

Requirements

  • High school diploma or equivalent
  • 3+ years of experience in Scheduling and Coordinating Healthcare appointments within a care facility (i.e., Skilled Nursing Facility (SNF), Long-Term Care...etc.)
  • Strong understanding of long-term care and provider workflows and the ability to manage multiple provider priorities simultaneously
  • Familiarity with Electronic Health Records (EHR) and Microsoft Office Suite
  • Excellent communication skills with the ability to interact professionally with clinicians, residents, and families
  • Ability to work on-site 5 days a week in the Lincolnwood, IL office (9:00 AM – 5:00 PM) with occasional local travel as needed (i.e. off-site training)

Nice To Haves

  • Associate or Bachelor's degree
  • Previous Healthcare administrative, finance, or operations experience within care facilities (i.e., Skilled Nursing Facilities (SNF), Hospice Care, Physical Therapy, Home Help or Hospitals)
  • Understanding of CMS regulations, medical terminology, HIPAA, and interdisciplinary care planning
  • Specific Experience within Skilled Nursing Facilities (SNF) or senior care environments, HIPAA compliance, and the Illinois MEDI system

Responsibilities

  • Create ad-hoc appointments for residents and provide real-time updates on medical orders and Provider arrival times to keep all stakeholders informed
  • Ensure facilities maintain real-time visibility into daily provider schedules and upcoming resident appointment
  • Accurately document appointment schedules, updates and communications in the facility's EHR and scheduling system inclusive of scheduling activities to meet regulatory timelines (i.e., 30-day, 60-day, and 90-day follow-up requirements for long-term care)
  • Serve as the primary point of contact for onsite providers (podiatry, dentistry, etc.) and communicate "provider census" lists to facility leadership
  • Accurately document schedules in the facility's EHR system, ensuring all activities meet regulatory timelines (30/60/90-day requirements) and HIPAA standards
  • Monitor and resolve scheduling conflicts that may impact nursing workflows, resident routines, or provider rounds
  • Partner with the Facility staff, social services and Medical Records to support admission scheduling for new residents and coordinate specialty, telehealth, or preventive care clinics
