Summary We are seeking a dependable and compassionate Healthcare Coordinator to serve as the vital connection between participants, healthcare providers, and internal nursing staff. In this hands-on role, you will help ensure participants receive timely medical care, accurate follow-up, and strong advocacy throughout their healthcare journey. Reporting to Health Services leadership, you will coordinate appointments, transport participants to medical visits, assist during consultations and procedures, and maintain accurate communication across care teams. This role offers an opportunity to directly improve participant health outcomes while building meaningful relationships with individuals, families, and community providers.

Requirements

High school diploma or equivalent required
Valid driver’s license from Illinois, Indiana, or Wisconsin with a clean driving record
Ability to communicate effectively with physicians, nurses, office staff, participants, and families
Strong organizational skills with the ability to manage schedules, records, and multiple priorities
Strong attention to detail and accuracy in documentation
Ability to remain calm, compassionate, and solution-focused during stressful situations
Willingness to provide care involving potential exposure to illness while following infection control protocols
Physical ability to assist participants with mobility needs during appointments and transportation
Basic computer skills including email, scheduling systems, data entry, and electronic documentation
Ability to maintain confidentiality and handle sensitive medical information appropriately
Ability to work independently while contributing to a collaborative healthcare team

Nice To Haves

Experience supporting individuals in healthcare, human services, caregiving, or care coordination settings preferred
Experience working with adults with developmental disabilities preferred but not required

Responsibilities

Coordinate and schedule medical, dental, vision, behavioral health, and specialty appointments for participants
Transport participants safely to and from appointments throughout the local area
Accompany participants during appointments, procedures, and consultations to provide support and advocacy
Communicate participant concerns, symptoms, preferences, and healthcare needs to providers in a professional manner
Ensure follow-up recommendations, treatment plans, prescriptions, and referrals are communicated to internal nursing staff and relevant team members
Maintain accurate health documentation, appointment records, visit summaries, and care coordination notes
Build and maintain strong working relationships with physicians, clinics, hospitals, pharmacies, therapists, and community healthcare providers
Advocate for participant needs during appointments, hospital visits, emergency situations, and care transitions
Support medication pick-up coordination, medical supply needs, and follow-up scheduling as directed
Monitor appointment outcomes and assist with tracking preventative care, annual exams, screenings, and ongoing treatment needs
Assist participants with mobility needs during transportation and appointments as required
Follow infection prevention, confidentiality, HIPAA, and safety protocols at all times
Collaborate closely with nursing staff, residential teams, guardians, and internal departments to ensure continuity of care
Respond calmly and professionally in urgent or high-stress healthcare situations
Support occasional schedule flexibility for urgent appointments or participant care needs
