Summary We are seeking a dependable and compassionate Healthcare Coordinator to serve as the vital connection between participants, healthcare providers, and internal nursing staff. In this hands-on role, you will help ensure participants receive timely medical care, accurate follow-up, and strong advocacy throughout their healthcare journey. Reporting to Health Services leadership, you will coordinate appointments, transport participants to medical visits, assist during consultations and procedures, and maintain accurate communication across care teams. This role offers an opportunity to directly improve participant health outcomes while building meaningful relationships with individuals, families, and community providers.
