About The Position

Cooperative for Human Services, Inc. (CHS) is seeking a Registered Nurse to join our nonprofit human services organization as a Healthcare Coordinator. This is an opportunity to use your clinical knowledge in a meaningful, community-based role where you can build lasting relationships, guide and educate employees, improve healthcare practices, and positively influence the lives of people with developmental disabilities. This is not a traditional hospital or bedside nursing position. Instead, you will work across CHS programs and community settings, helping ensure that individuals receive safe, thoughtful, person-centered healthcare while living in their homes and participating in their communities.

Requirements

  • Current, unrestricted Registered Nurse license in the Commonwealth of Massachusetts.
  • At least four years of nursing experience.
  • At least one year of experience supporting individuals with intellectual or developmental disabilities.
  • Current MAP Trainer certification or the ability and willingness to complete required MAP training and maintain certification.
  • Strong clinical assessment and professional judgment.
  • Excellent communication, training, analytical, and interpersonal skills.
  • Knowledge of the healthcare needs of individuals with developmental disabilities.
  • Ability to respond calmly and effectively to medical emergencies and healthcare concerns.
  • Valid driver’s license and the ability to travel independently to CHS program locations and other community settings.
  • Ability to work a flexible schedule and participate in required on-call coverage.
  • Ability to lift up to 30 pounds, with reasonable accommodation as applicable.

Nice To Haves

  • Current MAP Trainer certification is preferred.

Responsibilities

  • Coordinating and monitoring healthcare services for individuals supported by CHS.
  • Reviewing healthcare records, identifying emerging needs, and recommending appropriate services, testing, equipment, and follow-up care.
  • Providing clinical consultation, support, and guidance to employees across CHS programs.
  • Serving as one of CHS’s Medication Administration Program (MAP) Coordinators.
  • Supporting compliance with MAP regulations, policies, procedures, and documentation requirements.
  • Providing required MAP training and other health-related education to CHS employees.
  • Visiting program locations to audit healthcare records, monitor service delivery, and evaluate medication administration practices.
  • Collaborating with individuals, families, guardians, healthcare providers, CHS employees, DDS, regulatory bodies, and community partners.
  • Attending medical appointments, consultations, and Individual Support Plan meetings as needed.
  • Advocating for the healthcare needs, rights, dignity, and informed participation of each person supported.
  • Helping develop and improve healthcare and medication administration policies and procedures.
  • Participating in quality-improvement initiatives and preparing accurate and timely reports.
  • Recognizing and responding appropriately to potential abuse, neglect, safety, or healthcare concerns.
  • Ensuring that CHS programs have access to clinical guidance when it is needed, participation in on-call coverage is a required part of this position.
  • Providing consultation and support for health, medical, and safety concerns outside regular business hours when assigned on-call.
  • Assessing situations by phone, guiding employees through urgent concerns, and determining when additional medical intervention or emergency care is necessary.
