Safely transport individuals receiving support from Unity Bay to medical appointments. Provide physician with medical information needed during appointments. Assist individual with understanding medical condition and communicate the information to appropriate staff. File Medical Health Records and other medical information in assigned locations. Schedule and take individuals to medical, specialty, lab and other appointments as needed. Behavior should be professional at all times and consistent with Agency’s mission and policies and procedures, including treating individuals/consumers with dignity and respect. Exhibit appropriate work behavior with individuals receiving services, co-workers, families, consultants and other professionals in all settings. Provide the prescribed level of supervision and accountability throughout the individual’s daily routine. Follow proper lifting techniques to ensure proper care of individuals who are non-ambulatory. Record understandable notes in various Therap modules and paper reports as required as specified in the individual’s plan. Implement Behavior Support Programs, as written, to include documentation of all behavioral incidents. May require physical intervention and the use of Crisis Prevention techniques. Provide medications and treatments as ordered following Agency procedures, complete MAR, follow procedure to dispose expired medications and inform the nurse of shortage of medications, med errors, and need to give over-the-counter medications when working in facilities not staff by a nurse that is assigned specifically for consumer medication assistance. Maintain confidentiality of consumer information and personal passcodes. Be able to flex schedule and work multiple locations. Complete job duties related to a Direct Support Professional job description as needed. Other duties as assigned.
