At Cooper University Health Care, the Healthcare Access Navigator plays a crucial role in ensuring patients receive timely and coordinated care. The position involves appointment scheduling for patients with their Primary Care Provider (PCP) within 7 days or upon discharge, and assisting patients without a PCP in obtaining one and scheduling appropriate appointments. Navigators facilitate patient discharge from the 24/7 Emergency Department by coordinating and scheduling follow-up appointments. They work closely with primary care and specialty physicians to coordinate patient care plans, including consultations, procedures, testing, surgeries, or other services ordered by the treating ED provider. The role also includes verifying insurance coverage and demographic information, collecting patient payments, and retaining patients within the Cooper Health System by scheduling directly or through Central Scheduling. Navigators schedule appointments with community services when appropriate, collaborate with multidisciplinary teams, inform patients of billing policies and managed care requirements, and assist with obtaining referrals. They provide support and counseling to patients and families, communicate with referring providers, and partner with various resources to deliver well-coordinated, timely, and exemplary care. The position serves as a resource for community services, tracks and reports weekly on scheduled services, and requires flexibility to float to other ED Registration work areas as needed. This role covers discharged patients across various hospital-based care areas including IP units, CDU, Urgent Care Centers, Cooper Camden ED, and Salem ED.
