Managed by Momentum Senior Living, The Whitford provides a unique opportunity to join the founding leadership team and help build an exceptional community from the ground up. We are seeking an experienced, compassionate, and collaborative Health & Wellness Director to lead our clinical team and establish a culture of excellence in resident care. The Health & Wellness Director (Director of Nursing) is responsible for the overall leadership, management, and quality of nursing services throughout the community. This includes overseeing resident care, supervising clinical staff, ensuring regulatory compliance, managing departmental operations and budgets, and partnering with physicians, families, and the leadership team to deliver outstanding care. This leadership role is responsible for creating a supportive, resident-centered environment while maintaining the highest standards of clinical excellence.

Requirements

  • Graduate of an accredited school of nursing.
  • Current Licensed Vocational Nurse (LVN) license in the State of California.
  • Minimum of two (2) years of nursing administration or leadership experience, preferably in senior living, assisted living, memory care, or long-term care.
  • Minimum of two (2) years of geriatric nursing experience preferred.
  • Experience with resident assessments, care planning, quality assurance, regulatory compliance, and clinical operations.
  • Strong leadership, organizational, communication, and critical thinking skills.
  • Demonstrated ability to lead teams, manage multiple priorities, and build positive relationships with residents, families, physicians, and staff.
  • Basic computer proficiency and experience with electronic health records.

Nice To Haves

  • Bachelor's Degree in Nursing (BSN) preferred.
  • NADONA certification is a plus.

Responsibilities

  • Direct the overall operation and delivery of nursing services for Assisted Living and Memory Care residents.
  • Ensure residents receive individualized, high-quality care that promotes dignity, independence, and well-being.
  • Coordinate nursing services with physicians, home health agencies, hospice providers, rehabilitation services, private duty caregivers, pharmacies, and other healthcare professionals.
  • Evaluate resident admissions to ensure appropriate level of care and service planning.
  • Oversee resident assessments, care planning, clinical documentation, and ongoing care coordination.
  • Maintain clinical competency and remain current on best practices in geriatric nursing.
  • Recruit, hire, train, coach, mentor, evaluate, and develop nursing staff.
  • Maintain staffing schedules that provide quality resident care and meet all state and federal staffing requirements.
  • Coordinate orientation programs, annual competencies, continuing education, and in-service training.
  • Designate charge nurses and provide leadership coverage during absences.
  • Foster a positive, accountable, and collaborative work environment focused on quality care and employee engagement.
  • Maintain strong relationships with residents and families through regular communication and visibility throughout the community.
  • Ensure significant changes in resident condition are communicated promptly to physicians, families, and appropriate regulatory agencies.
  • Participate in resident care conferences and interdisciplinary meetings.
  • Attend resident and community events to strengthen resident engagement and relationships.
  • Ensure compliance with all federal, state, local, and company regulations, including HIPAA privacy standards.
  • Oversee completion and timely submission of clinical documentation, assessments, and MDS requirements as applicable.
  • Participate in Quality Assurance and Performance Improvement (QAPI) meetings and implement action plans.
  • Maintain nursing policies, procedures, standards of practice, and departmental manuals.
  • Ensure nursing personnel maintain current licenses, certifications, and required training.
  • Routinely audit clinical documentation and medical records for accuracy and compliance.
  • Manage departmental budgets and monitor monthly expenditures.
  • Oversee purchasing and inventory of nursing supplies, medical equipment, and office supplies.
  • Review operating reports and explain budget variances as needed.
  • Collaborate with the Executive Director to support community operations, occupancy, and resident satisfaction.
  • Participate in an on-call rotation for evenings, weekends, and holidays.
  • Serve as Manager on Duty on scheduled weekends.
  • Respond appropriately to clinical emergencies and operational issues as needed.
