About The Position

Fort HealthCare is seeking a Health Unit Coordinator to join their Emergency Services team. This role is crucial in collaborating with the clinical team to coordinate all aspects of patient care and transitions out of the hospital setting. The Health Unit Coordinator is responsible for performing duties related to patient admission, transfer, and discharge, communication with patients, guests, providers, and staff, electronic health records documentation, and other clerical duties. A key aspect of this position is the ability to maintain effective and efficient workflows under stressful conditions and periods of high unit activity, while utilizing effective and respectful communication with patients, families, and the health care team.

Requirements

  • High School Diploma or GED required
  • Ability to communicate professionally & appropriately with care team, patients, and family members
  • Excellent interpersonal skills
  • Adaptable to change
  • Ability to prioritize tasks and effectively manage time
  • Ability to self-motivate, work independently and function productively as a team member
  • Must possess keyboarding skills to perform accurate and detailed data entry
  • Demonstrates exemplary telephone etiquette
  • Familiarity with medical terminology
  • BLS Certification

Responsibilities

  • Serves as a link between patients, staff, providers, ancillary departments, and nursing staff.
  • Updates medical records and documents patient information, in collaboration with the care team.
  • Performs clerical functions pertaining to the needs of the department.
  • Orders & monitors supplies, assists with inventory, and other clerical duties per the needs of the department.
  • Maintains patient records in accordance with the status of the patient while working with computers efficiently and accurately.
  • Schedules appointments, as per department needs, and assists with patient admissions, transfers, and discharges.
  • Answers phones, communicates with patients, families, and staff, and facilitates communication between units.
