The Orange County Health Department has 2 openings for a Health Social Worker II. The work involves the responsibility for providing professional social work services for the Care Management for High Risk Pregnancies (CMHRP) program and the Care Management for At-Risk Children (CMARC) program with the Orange County Health Department. An employee in this class is responsible for provided services which include assessment, intervention, client education and high level comprehensive and intensive care management services to a limited caseload of eligible at-risk pregnant women and high-risk children (ages 0-5 years) with complicated personal, psycho-social, medical, developmental and economic needs. Work is performed under the general supervision of the Home Visiting Services Supervisor with some leeway to exercise independent judgment within the framework of applicable rules, regulations, policies and procedures in the performance of duties, both in terms of planning and accomplishing work and in making decisions about public health needs and requests.
