The Health Services Care Manager, within the framework of the Aging Service Access Points (ASAP) regulations, determines program eligibility and provides a consumer centered approach to assessment, service acquisition, reassessment, and monitoring of the services for the state home care and specialized MassHealth funded programs, to assist elders to live independently in the community. This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer a 4-day work week and hybrid work schedule (once training is complete.) No nights or weekends. The starting annual salary range is $50,872.46 - $54,804.09. A 6% increase will be used to recognize the required bilingual skill set in Russian, Spanish, or Haitian Creole that is required for this role.
