Founded in 1793, the Baltimore City Health Department is the oldest, continuously operating health department in the country. During the more than 220 years since then, we’ve been working to improve the health and well-being of Baltimore residents. The Health Department has a wide-ranging area of responsibility, including acute communicable diseases, animal control, chronic disease prevention, emergency preparedness, HIV/STD, maternal-child health, restaurant inspections, school health, senior services, and youth violence issues. The agency includes a workforce of approximately 800 employees and has a budget of approximately $208 million. The Baltimore City Health Department is seeking a dedicated and dynamic individual for the role of Local Health Improvement Coalition Director. This person will report to the Chief Policy and Strategy Officer and oversee the Local Health Improvement Coalition (LHIC) and various community health projects. The LHIC is committed to enhancing the health and well-being of our community through collaborative efforts and evidence-based strategies. The LHIC Director will play a pivotal role in guiding the coalition's activities, fostering partnerships, and implementing initiatives to address key health issues. The LHIC re-launched in 2021 and is composed of 120+ community, nonprofit, healthcare, and government members. The LHIC is at a critical and exciting point. We are executing on the LHIC’s current health priorities, which are diabetes, care coordination, and social determinants of health. The current efforts of the LHIC include 4 key strategies: (1) Educating and enrolling people in Diabetes Prevention Programs (DPP), (2) Developing multi-directional referral systems, (3) Finding solutions for CHW reimbursement, and (4) Supporting Diabetes workforces to address the social determinants of health. The LHIC is also developing the City’s Community Health Improvement Plan, which will drive our priorities for the next 3-5 years. To ensure the success of these efforts, the LHIC Director will oversee a team of 4 working to regularly convene multi-sector stakeholders, equitably engage community members, facilitate the development of shared goals and priorities, and monitor and track progress. The right candidate for this role will be experienced and skilled at centering community, have a strong background in program development and strategic planning, coalition development, and ability to work with and lead interdisciplinary initiatives. Experience with social needs/social determinants of health programs, technology solutions, and/or public health outreach will also be an asset.
