About The Position

Vail Health is seeking a Health Optimization Coach to join their Healthspan program. This role focuses on integrating health coaching with clinical care, providing personalized exercise programming, nutrition guidance, and lifestyle change facilitation to enhance patient health outcomes. The contractor will report to the Health Coach Program Director and will work within a state-of-the-art healthcare facility in Vail.

Requirements

  • Five years of experience in health coaching.
  • Strong knowledge of lifestyle medicine principles, exercise science, and nutrition.
  • Exceptional communication and interpersonal skills with the ability to inspire and motivate individuals.
  • Tech-savvy with the ability to utilize electronic health coaching and telehealth platforms.
  • Demonstrated ability to work collaboratively in a multidisciplinary team environment.
  • Familiarity with motivational interviewing and behavior change theories.
  • Ability to operate in a dynamic healthcare setting, requiring both in-person and occasional remote work.
  • Flexibility and adaptability.
  • Must have working knowledge of the English language, including reading, writing, and speaking English.

Nice To Haves

  • Certification from a recognized health coaching program (e.g., NBHWC, FMCA, IIN, or equivalent).
  • Bachelor’s degree in health sciences, nutrition, psychology, or a related field.

Responsibilities

  • Conduct consultations with program participants to understand their health histories, lifestyle, goals, challenges, and areas for improvement.
  • Educate patients on program goals, resources, and services, ensuring clear expectations and engagement.
  • Develop personalized health action plans including goal-setting, behavioral change strategies, nutrition, exercise, stress management, sleep hygiene, and accountability measures.
  • Act as a guide and motivator, empowering clients to adopt healthier lifestyles, manage chronic conditions, and improve overall well-being.
  • Collaborate with Healthspan clinicians to align health coaching efforts with patient care plans.
  • Design and implement safe, effective, and individualized fitness programs to support longevity, mobility, and strength.
  • Follow standardized processes/protocols for healthspan coaching and patient interactions.
  • Facilitate one-on-one and group coaching sessions and workshops.
  • Incorporate feedback from clinicians into coaching practices.
  • Monitor and evaluate clients' progress toward health goals, providing feedback and adjusting strategies.
  • Provide accountability, guidance, encouragement, and practical tools for health behavior changes.
  • Collect and provide patient data to measure program success (adherence rates, health outcomes, satisfaction scores).
  • Maintain accurate client documentation in accordance with program standards and legal requirements.
  • Participate in regular team meetings and multidisciplinary case reviews.
  • Share observations and patient feedback to make recommendations for program improvements.
  • Participate in community engagement events, including educational workshops and lectures.
  • Stay informed of best practices in health coaching, behavioral change, and lifestyle medicine.
  • Model the principles of a Just Culture.
  • Perform other duties as assigned.
  • Maintain HIPAA compliance and compliance with all applicable laws, regulations, and organizational policies.
