About The Position

Under the supervision and guidance of the Health/Nutrition Coordinator, the EHS Health/Nutrition Facilitators work to implement high-quality health, oral health, and nutrition services to all children, including services to children with disabilities, that are developmentally, culturally, and linguistically appropriate services. The EHS Health/Nutrition Facilitator assists the Health/Nutrition Coordinator in implementing nutritional services that are culturally and developmentally appropriate, meet the nutritional needs of and accommodate the feeding requirements of each child, including children with special dietary needs and children with disabilities.

Requirements

  • High School diploma or GED
  • 45 DCF Hours and Part 1 and 2 exams (must be obtained within DCF mandated timeline)
  • Able to stand or sit for extended periods
  • Able to lift 40 pounds

  • Knowledge in Head Start Performance Standards in relation to Health and Nutrition Services and regulations with emphasis on Early Head Start age.
  • Training in Medication administration in the classroom.
  • Training in USDA procedures for Child Care Food Program and Nutrition Training.

Responsibilities

  • Share culturally linguistically and developmentally appropriate, science-informed information regarding health, oral health, and nutritional needs of children, including appropriate information regarding children with disabilities, with families.
  • Know current pediatric periodicity schedules (EPSTD) and monitor the health database and files to ensure children (birth to five) are receiving the appropriate care.
  • Maintain confidentiality policies and procedures in accordance with HSPPS 1303 Subpart C.
  • Conduct ongoing monitoring to ensure the health and safety and nutritional needs of children, including children with disabilities, are met and that any needed improvements are implemented as quickly as possible.
  • Obtain determinations from health care and oral health care professionals as to whether or not the child is up-to-date on a schedule of age appropriate preventive and primary medical, mental health, and oral health care, based on: the well-child visits and dental periodicity schedules as prescribed by the Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) and immunization.
  • Assist parents with making arrangements to bring the child up-to-date as quickly as possible; and, if necessary, directly facilitate provision of health services to bring the child up-to-date with parent consent.
  • Ensure enrolled children have up to date health documents and physical files and Child Plus database are accurate to include physical exams, laboratory work, immunizations and dental exams (Head Circumference included).
  • Ensure Parent Notification Forms are distributed when health status items are due to expire 30 days before, 15 days previously. Create a “Health Review” event in Child Plus once a parent has been notified.
  • Provide follow-up for the children to receive the appropriate monitoring for health, dental, elevated lead levels and/or abnormal hearing or vision results.
  • In conjunction with the Health/Nutrition Coordinator, organize health screenings for children.
  • In coordination with the primary physician, or specialist, complete the IHP for children who have health problems and need medication in classrooms; keep the Family Service component updated in order to maintain accurate and up-to-date documentation.
  • In collaboration with ERSEA, ensure that within the first 30 calendar days after a child first attends the program, consult with parents to determine whether a child has ongoing sources of continuous, accessible health care - provided by a healthcare professional that maintains the child’s ongoing health record and is not primarily a source of emergency or urgent care.
  • Identify uninsured children enrolled in the program and link them to health insurance and/or to necessary medical and dental services.
  • Within 45 calendar days after the child first attends the program or, for the home-based program option, receives a home visit, a program must either obtain or perform evidence-based vision and hearing screenings.
  • Ensure appropriate oral hygiene procedures are implemented for every enrolled child in the EHS program. Dental hygiene practices will vary according to the age and developmental stage of each child.
  • Provide support to the Health/Nutrition Coordinator in maintaining the inventory of the Health Supplies.
  • In collaboration with education staff, maintain first aid supplies when notified of deficiencies. Inventory first aid kits on a monthly basis.
  • Identify and form new partnerships in the community in order to enhance the services offered by the Health and nutrition component.
  • Ensure all centers maintain health, medication, and safety records according to program requirements.
  • Provide support to the Health/Nutrition Coordinator during health and safety monitoring.
  • Attend and take minutes for the Health and Safety Committee meetings.
  • Assist with the coordination of emergency practice drills and report any incidents on a timely manner.
  • Assist the Health/Nutrition Coordinator with the coordination and operations of the mobile health bus.
  • Monitor supplies, equipment and materials on the health bus and request to the Health/Nutrition Coordinator as needed.
  • Pick up and drop off children from the classroom when a parent opts in to use telehealth.
  • Ensure health mobile bus sign in/out log is filled out.
  • Ensure that all Health and Mental Health Mobile Bus Consent form and documentation are obtained prior to services being provided.
  • Additional tasks assigned by the administration or the Health/Nutrition Coordinator.
