About The Position

The Health Navigator will play a vital role in supporting patients of SAC Health by guiding them through the complexities of their medical, mental, and social healthcare needs. This position will provide comprehensive assistance to ensure patients can access critical resources, including scheduling appointments, arranging transportation, connecting with community services, and accessing health education and preventative care. Health Navigators will also focus on addressing Social Determinants of Health (SDOH), offering support in identifying needs, assisting with applications, conducting interviews, and helping patients secure the resources necessary to improve their overall health and well-being.

Requirements

  • High School Diploma or GED required.
  • Current CPR/BLS certification (must be American Heart Association or Red Cross accredited program) required.
  • As a requirement of this position, you must receive EPIC certification for the module you have been hired into.
  • 3+ years of experience in community-based setting required.
  • Must have strong technical skills and simultaneously utilize multiple platforms, including Electronic Medical Records, WebEx Contact Center, Care Management, and database systems.
  • Work in a team to accomplish specific goals, new procedures, demonstrate use of equipment/supplies, communicate by phone/in person with department staff and outside sources.
  • Display assertiveness and leadership capabilities, effectively balancing administrative duties, client needs, and task management.
  • Be highly organized, detail-oriented, and proficient in presentation and communication skills.
  • Able to perform basic mathematical calculations and handle multiple duties simultaneously.
  • Strong organizational and problem-solving skills.
  • The ability to work well under pressure and maintain a calm demeanor.
  • Must be legally authorized to work in the United States on a full-time basis.
  • Must not now or in the future require sponsorship for employment visas.

Nice To Haves

  • Bachelors degree in Social Work or related field preferred.
  • Community Health Worker Certification preferred.
  • Healthcare experience preferred.

Responsibilities

  • Monitor and respond to incoming referrals for health navigation services, ensuring patients receive prompt and appropriate follow-up care to address identified needs.
  • Conduct psychosocial intakes to identify service needs and provide appropriate resource linkage.
  • Evaluate patient social determinants of health and identify appropriate resources. Assist patients is accessing and securing resources.
  • Evaluate patient needs and support systems and link to appropriate integrated care teams.
  • Assist patients in securing and maintaining health care coverage including assisting with Medi-Cal applications or linkage to managed care services.
  • Assist patients in the coordination of care across the health care continuum. This may include scheduling, referrals, transportation, and resource linkage.
  • Maintain a working knowledge of available resources, both internal and community-based.
  • Work as a liaison between the clinical team and community resources. Facilitate the connection of appropriate resources.
  • Provide patients with tailored health education and coaching, under the direction and guidance of the clinical team, to support the effective management of chronic diseases.
  • Provide education to individuals and families surrounding chronic conditions, preventative care, and social determinant of health.
  • Develop individualized care plans addressing patient needs and goals. Monitor and reevaluate patient progress as necessary.
  • Collaborate with patients to develop goals and interventions necessary for positive health outcomes.
  • Partner with providers, care management teams, and other health care team members to help support overall quality objectives.
  • Participate in organization and community-based events to foster connections and outreach.
  • Others duties as assigned, as needed to support organizational goals.
