We are currently seeking a compassionate licensed Clinician for the Health Integration Services Team (HIT). Health Integration Services supports adults and children who are not currently connected to a full array of mental health services, helping bridge gaps and improve access to timely, effective services. This position plays a vital role in ensuring clients receive the right care at the right time by coordinating follow-up appointments for clients recently hospitalized, seen in an emergency department, or discharged from substance use treatment. When clients are unable to connect with their established treatment team, this role provides meaningful follow- up support either in the office or in the client’s home. Working collaboratively with multidisciplinary teams across the agency, this position helps support clients across the lifespan. This position will also conduct clinical assessments, engage clients in safety planning, provide timely clinical intervention during crisis situations and offer follow-up care to promote stability and continuity of services.
