This role involves analyzing, sequencing, and validating assigned codes based on medical record documentation. The Health Information Coder will utilize automated encoders, coding books, and compliance resources to ensure accurate coding for patient information. A thorough understanding of coding rules, anatomy, physiology, and medical terminology is essential. The position requires reviewing all medical record documentation to assign diagnoses, procedures, level codes, and modifiers, and selecting the appropriate reimbursement grouper. The coder will ensure compliance with coding regulations and reimbursement requirements, abstract and enter patient information into relevant systems, and query physicians for clarification or additional documentation. Responsibilities also include reconciling and retrieving medical records, maintaining a high accuracy rate, achieving productivity standards, and managing workflow for timely claim submission. The role supports the orientation of new personnel, requires ongoing professional development to maintain certification, and staying updated on health information management advancements. Continuous monitoring of documentation, coding systems, and department workflow is expected to identify and resolve issues, communicating with the Coding Manager to implement improvements. The position also involves performing other assigned duties and interacting with others according to hospital customer service standards, including providing service excellence in line with Cape Cod Hospital’s CARES values.
