About The Position

Cornerstone Family Healthcare is actively recruiting for a Health Home Program Supervisor to join our growing team in Newburgh, NY. This role is responsible for the oversight of care management and outreach activities for individuals with chronic illnesses, including HIV, to engage them into the Cornerstone Health Home Program. The supervisor will complete enrollments, establish and maintain relationships with healthcare providers for referrals, and serve as a point of contact for co-location projects. A key responsibility is directly supervising Care Management staff, ensuring program outcomes are met, providing quality oversight of client charts in the EHR, and ensuring adherence to billing standards. The Program Supervisor may also carry a caseload as needed and perform Care Manager duties during staffing shortages.

Requirements

  • Master’s degree in social work, counseling or related field.
  • Some experience in human services.
  • Experience in a supervisory position OR BSW or equivalent degree with 1-3 years of experience in a supervisory position.
  • Well organized.
  • Outstanding oral and written communication skills.
  • Ability to manage time and multiple tasks efficiently.
  • Familiarity with non-profit and/or human service agency operation.
  • Sensitivity to HIV/AIDS issues.
  • Ability to work independently.
  • Proficient in Microsoft Word and Excel.
  • Must be able to provide immunization records for purpose of working with clients in hospitals/clinical settings.

Nice To Haves

  • Experience with electronic data entry.
  • Bilingual (English/Spanish).
  • Some flexibility to work early evening hours.
  • May work periodically throughout the week in an Emergency Room setting.

Responsibilities

  • Oversight of care management and outreach activities for individuals with chronic illnesses.
  • Conducting outreach services each month, which can include mailing letters, internet searches, cold calls, and field visits.
  • Completing enrollments with eligible members in their home and/or in clinical settings.
  • Establishing and maintaining collaborative relationships with hospitals and medical providers to facilitate ongoing referrals.
  • Serving as the point of contact for co-location projects.
  • Directly supervising Care Management staff, ensuring program outcomes are met.
  • Providing quality oversight of client charts in the EHR and ensuring adherence to billing standards for care management.
  • Carrying a caseload as needed and performing Care Manager responsibilities during staffing shortages or transitions.
  • Meeting and maintaining program billing and productivity standards.
  • Maintaining Electronic Health Record and all required electronic data and completing all program standards documentation.
  • Completing EHR and paper chart audits to ensure charting compliance.
  • Providing weekly supervision to CM staff, and other staff as assigned.
  • Providing Case Management services to an assigned caseload, on an as-needed basis.
  • Participating in supervision and program review each week with the VP of Health Homes.
  • Providing supportive training to all direct reports and department staff as necessary.
  • Attending and participating in monthly department, All Staff and other required meetings.
  • Being familiar with Cornerstone policies and procedures and the Employee Handbook.
  • Maintaining confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
  • Performing other related duties as assigned.
