Requirements

• Bachelor's degree in any field with at least three years of human services experience, OR Master’s degree in a human services field with at least one year of experience, OR CASAC certification with two years of human services experience.
• Proficiency in Microsoft Office.
• Reliable transportation and a valid New York State driver’s license required.
• Ability to work effectively in a fast-paced environment.

Nice To Haves

• Prior experience with the Medicaid delivery system preferred but not required.

Responsibilities

• Work collaboratively with participants to assess health goals and implement individualized Plans of Care.
• Complete HARP (Health and Recovery Plan) assessments to connect members with specialized community-based services.
• Support members in overcoming barriers to healthcare by providing health literacy education and addressing social determinants of health.
• Assist high-risk members in managing chronic behavioral health, medical, and substance use conditions.
• Link participants to appropriate health, behavioral health, and community resources.
• Coordinate with healthcare providers and community agencies to promote primary and preventive care.
• Assist participants in reducing or eliminating avoidable hospital and emergency room usage.
• Maintain and manage all aspects of participants Electronic Health Records (EHR).
• Promote a culture of inclusion, belonging, and respect.
