HEALTH HOME CARE MANAGER, HIGH ACUITY
About The Position
Care Managers provide outreach and enrollment services to children presumed to meet eligibility requirements for New York State's Children's Health Home program. They gather assessments and enter documentation to confirm eligibility and identify areas requiring supportive services. Working with children, families, and service providers, they develop a comprehensive Plan of Health Care, identify additional service providers as needed, and coordinate ongoing participation in carrying out the plan. Care Managers document progress and arrange transitions between health homes or to other levels of service when eligibility changes.
Requirements
- Bachelor's degree with 2 years of relevant experience, or a master's degree with 1 year of experience.
- Valid driver's license.
