About The Position

The Health Educator will be responsible for delivering self-management techniques and interventions for patients with multiple and chronic diseases, aiming for positive medical outcomes and patient care goals. This role requires knowledge of symptoms, treatments, and common chronic illnesses seen in primary care, and the ability to stay current on these topics. The Health Educator will also need to adapt quickly to changing conditions and new processes, and effectively communicate information through various means, including written reports and presentations. Additionally, the role involves designing, preparing, delivering, and modifying course curriculum and educational aids, as well as conducting literature searches. The position requires the ability to work in a multi-cultural setting and maintain cooperative relationships with colleagues, staff, administrators, and patients, functioning effectively as part of a professional healthcare team in a community setting.

Requirements

  • Bachelor’s degree in health education, health promotion, public health, social work, nursing, dietetics or related field.
  • Knowledge of public health problems, principles and practices.
  • Knowledge of theoretical concepts of health behavior and change theory.
  • Knowledge of current social and economic problems as they relate to public health.
  • Knowledge of basic epidemiology.
  • Ability to establish and maintain effective working relationships with patients, staff and the public.
  • Knowledge of multiple and chronic diseases.
  • Ability to deliver self-management techniques and interventions specifically geared towards positive medical outcomes and goals of the patients care.
  • Working knowledge of symptoms, treatments and common chronic illnesses as seen in primary care.
  • Ability to adapt quickly to changing conditions and assimilating new processes into job functions.
  • Knowledge of and ability to use software used in health care settings.
  • Ability to communicate clearly orally and in writing.
  • Ability to write reports, business correspondence, procedures.
  • Ability to correctly document patient progress.
  • Ability to effectively present information.
  • Adequate transportation to visit the sites of the WWFHC.
  • Demonstrate a history of cooperative relations with colleagues, support staff, administrators, and patients.
  • Excellent communication and listening skills, including motivational interviewing.
  • Ability to work in a multi-cultural setting.
  • Excellent teaching and communication skills.
  • Ability to organize and prioritize work appropriately.
  • Ability to design, prepare, deliver and modify course curriculum and associated educational aids.
  • Ability to do literature searches.

Nice To Haves

  • One (1) year experience working in a public health setting, community or public health oriented environment.
  • One (1) year of experience working with an electronic medical record system.
  • Bilingual (English/Arabic or English/Spanish).
  • Experience with Microsoft Office Suite products.
