About The Position

Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. The Health Educator will serve as a key member of the Family Health Centers at NYU Langone School-Based Health Center (SBHC) team and will be housed under the Project SAFE program. The role provides sexual and reproductive health education, HIV counseling and testing, risk reduction counseling, outreach, and linkage to care for adolescents in FHC SBHCs. The Health Educator promotes utilization of SBHC services through individual engagement, clinic tours, outreach activities, and collaboration with school and community partners. The position also supports the implementation of evidence-based sexual health education workshops in schools throughout Brooklyn.

Requirements

  • HS Diploma
  • Prior work experience with community health outreach.
  • Health related background.

Responsibilities

  • Provide sexual and reproductive health education to adolescents within Family Health Centers at NYU Langone School-Based Health Centers.
  • Conduct rapid HIV counseling and testing services and complete all required documentation in accordance with program and regulatory requirements.
  • Provide individualized risk reduction counseling to adolescents and connect youth to appropriate medical, behavioral health, and social support services.
  • Conduct Social Determinants of Health (SDOH) screenings with youth and facilitate connection to appropriate resources and supportive services based on identified needs.
  • Promote awareness and utilization of School-Based Health Center services through outreach, relationship-building with students, School-Based Health Center tours and other enrollment activities to increase student engagement and access to care.
  • Assist eligible patients with enrollment in the Family Planning Benefit Program and other applicable health coverage programs as needed.
  • Participate in SBHC morning huddles, care team meetings, and other interdisciplinary meetings as required.
  • Provide evidence based sexual health workshops in high school classrooms.
