About The Position

Meeting Street is seeking a Health Coordinator for its Early Head Start program. This role is responsible for supporting the health, growth, and development of enrolled children through home visits, health education, and referrals. The Health Coordinator will maintain health records, enter data, assist with reporting, and collaborate with staff to address health-related needs. Requirements

A minimum of 1 year of experience working with infants, toddlers and families in an Early Childhood setting.
Licensed as a Registered Nurse by the Board of Nurse Registration and Nursing Education – Rhode Island Department of Health.
CPR and first aid certifications or be willing and able to become certified.
Employees must have access to their own vehicle, with valid insurance and a valid driver’s license.

Nice To Haves

Home visiting and or case management experience within a multidisciplinary team preferred.
Early Intervention service is provided in the child’s Natural Environments, which include the home, childcare settings and the community or other immediate environments natural the child and family’s environment.
Meeting Street will provide an agency smartphone to maintain communication with Clients and other Meeting Street employees.
Able to get at child’s level, climb stairs, deal with weather conditions, lifting, bending, stooping.

Responsibilities

Conduct home visits to assess and monitor the health, growth, and development of enrolled children.
Provide health education to families, focusing on preventative care and addressing specific health concerns.
Make referrals to healthcare providers and community resources as needed and follow up to ensure services are received.
Maintain accurate and up-to-date health records for all enrolled children, ensuring confidentiality and compliance with program standards.
Enter and track health data in systems such as KidsNet and ChildPlus.
Assist with the preparation of required reports, including the Program Information Report (PIR).
Participate in case conferences and multidisciplinary team meetings to coordinate care for families.
Work with staff to identify and address health-related needs impacting children’s development.
