About The Position

Sun River Health is seeking a full-time Health Coach to provide self-management education and support to patients with chronic medical and behavioral health conditions. This role involves addressing social determinants of health concerns and engaging high-risk patients. The position is primarily remote but may require travel to local Sun River Health locations for trainings and/or meetings. Sun River Health is a Federally Qualified, Non-Profit Health Center dedicated to providing high-quality, comprehensive primary, preventative, and behavioral health services to all, especially the underserved and vulnerable, across multiple counties in New York.

Requirements

High School Diploma / GED required
2+ years relevant experience in healthcare is preferred
ECW experience preferred

Nice To Haves

Bachelors Degree is preferred

Responsibilities

Outreaches and engages high risk patients regarding their engagement with primary care.
Facilitates completion of self-management needs assessment with high-risk patients, including the social determinants of health screening tool.
Develops a self-management care plan and goals with patients.
Conducts sessions in person or via tele-video (in accordance with NYS regulations).
Provides each patient with a copy of their approved plan.
Implements self-management care plan with educational session(s).
Ensures providers have approved and signed all self-management plans.
Provides linkage, coordination with, referral to and follow-up with appropriate ongoing service providers, including mental health and medical specialists, utilizing Unite Us and Referral Point.
Documents notes and care plans timely, and with accuracy, in agency EMR (eCW).
Tracks/ monitors patient progress and produces/maintains detailed, accurate and timely case notes documented in agency EMR.
Provides ongoing engagement with patients, at a minimum of two times per year.
Other duties as assigned.
