About The Position
Join the Heart of Caregiving: Health Coach Specialist! Do you believe that home is where the heart (and the best care) is? We are looking for a high-energy, empathetic Health Coach to join our team and support our Structured Family Caregiving program. In this role, you aren’t just a coach; you’re a champion for independence, dignity, and family-centered care. You’ll help patients stay in their own homes by supporting the incredible caregivers who look after them every day.

- A Tech-Savvy Multitasker: You’re comfortable navigating software (Salesforce fans, step to the front!) and love the fast-paced energy of a startup.

- A HIPAA Hero: You treat patient privacy with the utmost respect.
Responsibilities
- Be the Field Expert: Visit homes to perform initial assessments and ensure a safe, cozy environment for our members.
- Coach & Connect: Build deep rapport with caregivers—guiding, educating, and supporting them through virtual and in-person check-ins.
- Problem-Solve Like a Pro: Partner with RNs and Case Managers to escalate concerns and find solutions for evolving health needs.
- Stay Organized: Keep everything running smoothly by tracking your magic in Salesforce.
- Travel & Flex: Work mostly from the comfort of your home, with periodic trips to our Connecticut offices to sync with the squad.
