About The Position

This is a fully remote, fixed-term contract role for a Health Coach (Dietitian/Nutritionist) specializing in weight management. The company is a health-tech firm aiming to tackle obesity and type 2 diabetes. The Health Coach will support individuals living with obesity in making sustainable lifestyle and behavior changes as part of a multidisciplinary team within the Specialist Weight Management Service (SWMS). The role involves delivering care via video consultations and in-app messaging, requiring flexibility for some evening and weekend work to support NHS contracts. The company has a strong track record of positively impacting lives and is a leading digital health provider to the NHS.

Requirements

  • Experience working in a weight management/bariatric/eating disorder setting.
  • A qualified HCPC-registered Dietitian or AfN-registered Nutritionist with experience of weight management and/or bariatric experience, or working with patients in an eating disorder setting.
  • Proven experience delivering group-based programmes, with the confidence to facilitate sessions effectively in a digital setting.
  • A solid understanding of obesity as a condition and the issues many people living with obesity face, including weight stigma.
  • Experience or training in health coaching, behaviour change, or motivational interviewing, with awareness of coaching models and approaches.
  • Able to provide supportive, empathetic patient-centred care to patients who may have complex physical and mental health needs.
  • Comfortable working fully remotely and able to stay motivated and organised without an office.
  • A suitable home working setup, including a private space for confidential consultations and a reliable internet connection.
  • Able to work autonomously and collaboratively with a doctor-led multidisciplinary team (MDT) to oversee patient care.
  • Confidently and independently manage a caseload of service users, delivering nutritional and lifestyle interventions via video calls and in-app messaging, and working collaboratively with our MDT.
  • Experience in supporting the coordination of a healthcare service.
  • Experience in monitoring and supporting patients' journeys through a service in line with an organisation's protocols, including scheduling assessments and appointments, monitoring attendance rates and following up with patients, writing GP letters, and revising protocols.
  • Experience using Excel or Google Sheets (or other similar programmes) to monitor patients' journeys.
  • Dynamic with a strong drive to work in healthcare tech.
  • Experience thriving in fast-paced environments and energised by the dynamic nature of start-up life, where being hands-on and adaptable to the evolving needs of the service is essential.
  • Confident in using technology to provide patient care, including recording and reviewing patient notes, and providing in-app coaching.
  • HCPC registration as a Dietitian, or AfN registration as a Nutritionist (Registered Nutritionist or Registered Associate Nutritionist).
  • Relevant degree in Dietetics, Nutrition or a related field.

Responsibilities

  • Independently implement appropriate initial assessment methods to allocate service users to the most suitable treatment pathway.
  • Deliver group-based and one-to-one nutritional and lifestyle interventions remotely via video calls, to support changes in dietary and physical activity behaviours.
  • Provide ongoing coaching and support between sessions through in-app messaging.
  • Support service users across both medicated and non-medicated programmes, including those accessing GLP-1 medication.
  • Attend regular MDT meetings to monitor the care of our service users.
  • Attend clinical supervision to discuss complex or high-risk cases.
  • Appropriately manage clinical risk (e.g. safeguarding concerns) when it arises during clinical practice.
  • Maintain adequate CPD, receive regular clinical supervision in accordance with good practice guidelines and continue to meet your registration requirements (HCPC or AfN).
  • Maintain confidentiality of information in line with the requirements of the Data Protection Act and ensuring that participant confidentiality is protected at all times.
  • Contribute to innovation projects, service evaluation, and ongoing programme development at Second Nature.
