About The Position

The patient navigator’s primary role will be to assist appointment scheduling and access and to coordinate diabetes care for families who are at high risk of diabetes complications.

Requirements

  • Bachelor’s degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
  • 1 year of professional experience working in quality improvement within a medical practice.
  • 2 years of professional experience working in quality improvement within a medical practice.
  • Substitution: A combination of education and related technical/military/paraprofessional experience may be substituted for a bachelor’s degree on a year for year basis.
  • An advanced degree (Masters or Doctorate) may be substituted for experience on a year for year basis if the degree is in a field of study directly related to the work assignment.
  • Applicants must meet minimum qualifications at the time of hire.
  • Ability to communicate effectively, both in writing and orally.
  • Ability to establish and maintain effective working relationships with employees at all levels throughout the institution.
  • Outstanding customer service skills.
  • Excellent organizational skills and the ability to manage multiple projects at a time.
  • Flexibility and the ability to adapt to various situations.
  • Strong organizational skills and attention to detail.
  • Ability to work independently as well as part of a team

Nice To Haves

  • Master of Public Health (MPH) or related advanced degree preferred
  • Experience in quality improvement in a type 1 diabetes clinic or clinical research environment
  • Oral Spanish language proficiency
  • Experience building and running reports in an electronic medical record, preferably the Epic system
  • Advanced knowledge of the Microsoft Office package including Word, Excel, PowerPoint, Access, and Outlook
  • Basic understanding of statistical and data analysis

Responsibilities

  • Identify families in need of patient navigator outreach
  • Outreach to the families via phone, email, portal messages, and in person in the clinic
  • Schedule patient visits
  • Set up Medicaid ride services
  • Refer diabetes management questions to the diabetes educator
