About The Position

This remote position, located in the Western Kentucky region (Madisonville, Paducah, Hopkinsville, etc.), is responsible for providing subject matter expertise in evaluating, supporting, and coordinating healthcare and care needs for Veterans. The role assists Veterans in gaining access to healthcare through VA or community providers, supports healthcare plans by identifying and resolving barriers to care, and provides education on wellness-related topics. The position requires regular travel, serving as a liaison between Veterans and healthcare and community care representatives. The SSVF Health Care Navigator will not provide direct healthcare services, mental health counseling, or make treatment recommendations.

Requirements

  • Bachelor’s degree in social work or health care administration, or related field with a LCSW, Master’s level social worker or equivalent education and experience preferred.
  • At least three years’ experience in care management of clients.
  • Experience with the VA Medical System preferred.
  • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
  • Ability to travel and transport clients to medical appointments.
  • Ability to read, to write, and communicate the English language.
  • Ability to keypunch information into a computer.
  • Ability to sort and file documents alphabetically and numerically.
  • Ability to operate and answer the telephone.
  • Ability to operate various general office equipment including a computer and scanner.
  • Ability to do basic business and office work.
  • Regularly required to talk and listen, stand, walk, sit and use hands.
  • Must be able to regularly lift up to 30 pounds and occasionally lift up to fifty pounds.
  • Use of Office and Computer Equipment is necessary.
  • Vision abilities will require close vision.
  • Valid State Driver’s License.

Responsibilities

  • Provides comprehensive case management and care coordination across episodes of care.
  • Serves as a health coach by proactively supporting the Veteran.
  • Conducts assessments of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members, and significant others to understand the Veteran’s situation, potential barriers to care, the causes, and the impact of such barriers on the Veteran’s ability to access and maintain health care services.
  • Highlights the Veteran's strengths, limitations, risk factors, and internal/external supports and service needs to optimize the Veteran's ability to access and maintain health care services.
  • Completes the initial assessment as specified by the policy of the SSVF grantee, which may be accomplished through virtual technology.
  • Participates in expanding the knowledge related to health care navigators and the Veteran population.
  • Assists in identifying the Veteran and family's health education needs and provides education services and materials that match the health literacy level of the Veteran.
