About The Position

This position is within the WRHA Community Health Services - Access St. Boniface - 170 Goulet Street, S5 St. Boniface Team HomeCare department. The role is part of the Clinical job stream and is unionized with CUPE. This is a term position to cover a maternity leave, with an anticipated start date of July 17, 2026, and an end date of June 4, 2027. The position is 0.70 FTE and involves a mix of day, evening, and weekend shifts. Weekday shifts are 8 hours, and weekend shifts are 4 hours. The work arrangement is in person.

Requirements

Completion of a recognized Health Care Aide (HCA) program or equivalent.
Current Basic Life Support (BLS) Heart Code - Level C certification.
Experience in a home care setting is an asset.
Ability to work independently and as part of a team.
Excellent communication and interpersonal skills.
Proficiency in documentation and record-keeping.
Valid driver's license and access to a reliable vehicle may be required.

Nice To Haves

Experience with electronic health records.
Additional certifications in specialized care areas.

Responsibilities

Provide direct patient care in a home care setting.
Assist clients with activities of daily living.
Administer medications as prescribed.
Monitor client health status and report changes to the appropriate healthcare professional.
Educate clients and families on health management.
Collaborate with the healthcare team to develop and implement care plans.
Maintain accurate and timely documentation.
